It's a service that supports individuals with developmental disabilities to become more independent by assisting with daily living needs within their homes and communities

Medical transport Ohio
Homemaker Personal Care Transportation

HPC transportation refers to the transportation services provided in conjunction with Homemaker personal care services. It's a way for individuals to get to and from activities with their Personal HPC provider, such as errands, appointments, or social engagements.

​Behavior Health Respite (BHC)

BHC is a short-term, temporary relief service for primary caregivers of youth (under age 21) enrolled in OhioRISE. Its purpose is to support and preserve the primary caregiving relationship by giving caregivers a needed break, while the youth is safely supervised.

Shared Living

Ohio shared living refers to a service where individuals with developmental disabilities receive support while living with a family member or caregiver. This support includes at least 20% of the person's personal care and support needs being provided by the family or caregiver. It's essentially a home-based service model that aims to help individuals with developmental disabilities live in a more community-integrated setting

Community Respite

​Community Respite is a short-term, out-of-home service that provides care for individuals with developmental disabilities in a certified community setting, such as a respite home, camp, or agency site. Its purpose is to give primary caregivers a break while ensuring the individual remains safe, supported, and engaged in meaningful activities

Community Transition

Community Transition supports individuals who are moving from long-term institutional settings—specifically an Intermediate Care Facility (ICF) or a nursing facility—into community-based living under the Individual Options (IO) waiver. These services are designed to cover one-time, necessary household setup costs that help establish a stable home environment.
