HPC Transportation is provided alongside our Homemaker Personal Care services — the same trusted caregiver who supports you at home safely gets you to medical appointments, work, errands, and community activities. Your caregiver doesn't just drop you off; they're with you the whole way.

Who Qualifies for HPC Transportation?

HPC Transportation may be right for you if you:
 

  • Receive Homemaker Personal Care (HPC) through a DODD waiver

  • Need reliable rides to appointments, work, errands, or activities

  • Want door-to-door support from a caregiver you already know and trust


Transportation is added alongside HPC in your service plan — your SSA or our team can help you coordinate it.

How to Get Started

Getting started is simple and supportive:
 

  • Confirm you're receiving HPC services through your DODD waiver

  • Ask your SSA to add transportation to your service plan

  • Choose Frontline Healthcare as your provider

  • Tell us where life takes you — appointments, work, activities

  • We match you with a caregiver-driver you know and trust

  • Enjoy safe, reliable rides with support the whole way
