What is OhioRISE?

​OhioRISE (Ohio Resilience through Integrated Systems and Excellence) is a specialized Medicaid program designed to support children and youth in Ohio with complex behavioral health and multi-system needs. It provides coordinated, wrap around care that addresses mental health, physical health, and social well-being—ensuring families receive comprehensive, individualized support.

Who Qualifies for OhioRISE?

 

OhioRISE is designed for individuals who:
 

  • Are under the age of 21

  • Are enrolled in Ohio Medicaid and have significant behavioral health needs or complex care challenges

  • Are involved in multiple systems (such as mental health services, child welfare, or developmental support services)


Eligibility is determined through an assessment process that evaluates the child’s level of need and the type of support required.

How to Get Started with OhioRISE

Getting started is simple and supportive:
 

  • Enroll in Ohio Medicaid (if not already enrolled)

  • Complete an OhioRISE eligibility assessment

  • Get connected with a Care Coordinator

  • Develop a person-centered care plan

  • Choose Frontline Healthcare as your provider

  • Once approved, services can begin based on your individualized plan and approved hours.
